Hospice Licensed Practical Nurse will be responsible for providing
skilled nursing services within the scope of the Nurse Practice
Act, (NPA) as outlined in the IDT plan of care in accordance with
the physician’s orders and under the supervision of the
Registered Nurse.

the Professional nurses’ direction, the HHA is responsible for

Triage
RN Fulltime
The
On-Call Triage RN provides continuity of care to patients and
families after regular office hours.

Case
worker
The
Caseworker is responsible for the planning, implementation,
functioning and evaluation of casework services needed by patients
and families. The Caseworker works closely with patient
care, admissions and other areas of finance assisting patients and
families with necessary applications for potential placements into
alternate residences.
