The Clinical Dietitian is responsible for the nutritional care and education of all patients/residents in accordance with current applicable federal, state, and local standards, guidelines and regulations, our established policies and procedures, through physician/nurse referral.

 

Bereavement Counselors FT

The Bereavement Counselor is a qualified counselor who is responsible for assessing and intervening in social emotional factors related to the bereavement process.

 

 

Chaplain – North Miami Dade Team

The Chaplain provides spiritual support to patients and families in cooperation with the Interdisciplinary Team’s plan of care.  Acts as community liaison for patients linking them to local congregation/parish/synagogue.

Field Registered Nurse FT - North Miami Dade Team
FT with benefit; The RN is responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the terminally ill patient and family unit.

 

Home Health Aid’s Continuous Care

The nursing assistants provides direct hands-on care and/or assisting patients under the direction of the registered nurse.  The nursing assistants should be able to respond sensitively to patient/family needs by observing and reporting any change to include physical, spiritual, social, and emotional.

 

Licensed Practical Nurses Continuous Care

The Hospice Licensed Practical Nurse will be responsible for providing skilled nursing services within the scope of the Nurse Practice Act, (NPA) as outlined in the IDT plan of care in accordance with the physician’s orders and under the supervision of the Registered Nurse.

 

 

Financial  Analyst - North Miami Dade Team

The Financial Analyst will assist the VP, Finance and Administration with all aspects of contract administration, correspondence and contract file maintenance, and other finance related projects as requested.

The Patient Care Secretary provides clerical support to patient care teams.

Weekend Hospice Aid

Under the Professional nurses’ direction, the HHA is responsible for providing care and/or assisting with the demonstrating that care when appropriate.  The HHA should be able to respond sensitively to patient/family needs by observing and reporting any change to include physical, spiritual, social, and emotional.

 

 

Triage RN Fulltime

The On-Call Triage RN provides continuity of care to patients and families after regular office hours.

Case worker

The Caseworker is responsible for the planning, implementation, functioning and evaluation of casework services needed by patients and families.  The Caseworker works closely with patient care, admissions and other areas of finance assisting patients and families with necessary applications for potential placements into alternate residences.


Catholic Hospice will be opening an Inpatient Unit in the fall of 2008.
