Dietician:
The Clinical Dietitian is responsible for the nutritional care and education of all patients/residents in accordance with current applicable federal, state, and local standards, guidelines and regulations, our established policies and procedures, through physician/nurse referral.
Field Registered Nurse FT - North Miami Dade Team:
FT with benefit; The RN is responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the terminally ill patient and family unit.
Licensed Practical Nurses Continuous Care - Miami Dade Team:
The Hospice Licensed Practical Nurse will be responsible for providing skilled nursing services within the scope of the Nurse Practice Act, (NPA) as outlined in the IDT plan of care in accordance with the physician's orders and under the supervision of the Registered Nurse.
Triage RN Fulltime
The On-Call Triage RN provides continuity of care to patients and families after regular office hours.
RN Weekend Visits:
The Weekend RN provides continuity of care to patients and families after regular office hours through telephone triage and coordinates patient visits during the weekend and makes visits whenever necessary.
Social Worker
The Social Worker is responsible for the planning, implementation, functioning and evaluation of the social services needed by patients and families.
Bereavement Counselors FT - North and South Dade Teams:
The Bereavement Counselor is a qualified counselor who is responsible for assessing and intervening in social emotional factors related to the bereavement process.
Patient care secretary - Miami Dade Team:
The Patient Care Secretary provides clerical support to patient care teams.
Team manager
The Team Manager is the leader of the Patient Care Team whose function is to: supervise, evaluate and coordinate the various members of the interdisciplinary team. The Team Manager assures continuity of care from admission to discharge or transfer to bereavement; serves as patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the team's patients; assumes responsibility for the maintenance of patient records from admission through discharge or transfer to bereavement. The Team Manager directly reports to the Director of Patient Care.
Helpdesk Technician
Office Service Coordinator:
Oversees office-related issues such as building maintenance, inventory, tenancy related issues, office equipment assembly and movement. The Office Coordinator processes and takes care of purchase orders from staff members printing requests; assists Director of A/P with monitoring, reporting and resolving building problems and tenancy related issues as well as potential or actual safety risks at the offices.

Home Health Aid Continuous Care:
The nursing assistant provides direct hands-on care and/or assisting patients under the direction of the registered nurse. The nursing assistant should be able to respond sensitively to patient/family needs by observing and reporting any change to include physical, spiritual, social, and emotional.
Admission RN:
The Admissions Nurse is responsible for evaluating referrals and coordinating patient/family needs for admission to the Hospice Program.
